Step  1:
Make sure you have installed drivers for your graphics adapter from the cards manufacturer, not the default windows drivers for your card.  Most cards will be either:

Intel:
http://downloadfinder.intel.com/scripts-df-external/Support_Intel.aspx?iid=homepageA+hdr_nav2_download" target="_blank">http://downloadfinder.intel.com/scripts....ownload
look under the graphics link for your graphics adapter.

ATI:
http://ati.amd.com/support/driver.html" target="_blank">http://ati.amd.com/support/driver.html
Select your operating system and card to get the correct driver download link.

NVIDIA:
http://www.nvidia.com/content/drivers/drivers.asp" target="_blank">http://www.nvidia.com/content/drivers/drivers.asp
Select Graphics drivers, your operating system and card to get the correct driver download link.

Make sure you follow the installation instructions to install the drivers correctly.  You will then need to restart your computer before running Gish.

Step  2 - DEMO ONLY:
Go to your "Gish Demo" folder and run the file oalinst.exe to install OpenAL drivers.  This is necessary for the sound and music and can cause Gish to slow to a crawl if not installed correctly.
