I think it is an SDL bug. It happens for me as well, since I am running Gish on my laptop with two screens attached. The main screen is 1280x768, the secondary is 1280x1024. It happens for other SDL games as well.

Just for the sake of completeness:
There is a secret warp zone in mario level which allows you to warp to levels 2-1, 3-1 and 4-1 respectively. It is located behind the exit tube, so you have to climb on top of the ceiling and continue walking to the right. There will be a room with three tubes.. the leftmost one brings you to 4-1. Very much mario style
